During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Water testing is in progress the center is on a waiting list.

Comments:
An unannounced, on-site renewal inspection was initiated on 5/27/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:40 and ended at 12:05. The inspector reviewed compliance in the areas listed above. There were 34 children present and 5 staff as well as the owner present at the time of the inspection. The inspector reviewed 6 children?s records and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.011-F
Description: The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The most recent violation notice is not posted at the facility.

Plan of Correction: I will make sure I print out the paperwork when I receive it and make sure the center has it posted.

Standard #: 22.1-289.035-A
Description: staff must have subsequent background checks done every 5 years.

One staff. staff 3, had the first Central registry check done on 12/18/29 and does have a
subsequent Central registry check in the file.

Plan of Correction: I realized that the staff needed a subsequent CPS check and sent it in 2 weeks ago.

Standard #: 22.1-289.035-B-2
Description: Criminal records check must be on file before a staff can start working at the center.

Staff 1 started working at the center on 5/18/2025 and is present at the time of the inspection. The center does not have a criminal records check on file.

Plan of Correction: I have sent it in on 5/18/25 and I am having issues getting the results back. I will make sure the staff will not be at the center before we get the background checks back.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

floor cleaner was stored on the floor by the backdoor in the childcare room.

Plan of Correction: I was taking the cleaners to my car but then the tour arrived I will remove.
Removed during the inspection.

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Authorization for one emergency medication expired on 5/7/25 and the medication has not been returned to the parent.

Plan of Correction: I will ask the parent for a new authorization for the medication or give the medication back to them.

Standard #: 8VAC20-780-560-G
Description: When food is brought from home it must be labeled with the name of the child and the date.

A bottle containing milk was found on the counter in the childcare room, the bottle did not have the name of the child it belonged to or the date.

Plan of Correction: It is a parent that brings in a bottle for one child, we will tell them to label it or not bring it in. I will label it right now. Corrected during the inspection.
