During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Comments:

An unannounced, on-site monitoring inspection was initiated and completed on May 22, 2025 as a part of probation. The Department issued a Special Order on June 25, 2024 to place the licensee on probation for a period of twelve (12) months. The Department issued a Notice of Intent to deny renewal application for a license on November 13, 2024 as a result of high-risk violations. The Department entered into a Consent Agreement with the facility on April 16, 2025.
The on-site inspection began at 10:55 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 51 children present and 8 staff. The inspector reviewed five children's records and two new staff record on-site. This inspection included document review, tour of the facility and interviews.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
