During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on January 17, 2025, and completed on January 17, 2025, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to areas of physical plant. The on-site inspection began at 3:00 pm and ended at 3:40 pm. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
