During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 5-28-25. The on-site inspection began at 12:50pm and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/9/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.011-F
Description: The Center is required to post the license in a conspicuous place on the licensed premises.

The license was posted inside a locked office not in a conspicuous place.

Plan of Correction: The past 10 years it been at the same place.

Will post it in a conspicuous place.

Standard #: 22.1-289.035-B-3
Description: (Repeat Violation) For staff hired after 7/1/24, the Provider is required to request a Central Registry check prior to employment.

The record for Staff #1 did not contain documentation that the Central Registry check had been requested prior to hire.

Plan of Correction: The central registry was submitted before hired on 2/7/25 and i had the receipt for it till i got the results I put the results and I just trashed the receipt as the results was clear and good and was no purpose just to keep it. this is just overwhelming with all those terms changes. I will make sure I just save the receipt/ note documents in their file for ever.

Standard #: 8VAC20-780-340-F
Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Upon arrival for the inspection in the Preschool classroom (ages 16 months-3 years), there was 1 staff member with 7 children resting on 1 side of a half wall that divides the room and there was no teacher with the 10 resting children on the other side of the half wall. The 10 children were alone and without sight and sound supervision.

Plan of Correction: 1) Staff mistake about students age we DONT have any 16 months students at the center.

2) It was naptime all the kids were sleeping, the other staff remembered the staff who went on her break left the vacuum outside so she ran to go and put it in the closet when the inspector came she saw her coming by the last step by the gate which was not even a minute and stayed with the other staff and the kids during nap time. which was her mistake she was not supposed to go and put the vacuum away it wouldve been fine if it stayed outside. the kids were not left alone as both staff were there during the rest of the Inspection while the inspector was there.

same day we held a staff meeting and will make sure no other staff leave the kids sight even for a second.

Standard #: 8VAC20-780-500-B
Description: The Center is required to ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member?s hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
The foot-operated storage system for disposal of diapers was not able to be operated by foot causing the staff member to have to touch the exterior surface of the trash can with their hand or a soiled diaper to open the trash can.

Plan of Correction: cover lid clip came off the staff after changing the kids she forgot to clip it back in.



Staff will make sure its always clipped and operated by foot
