An unannounced, on-site monitoring inspection was initiated on 6/2/2025 and completed on 6/2/2025, as a part of the licensure period. The on-site inspection began at 11:34AM and ended at 12:50PM. The inspector reviewed compliance in the areas listed above. There were 24 children present and 6 staff. The inspector reviewed 3 children?s records on site on 6/2/2025 and 2 staff records on site 6/2/2025. This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-70 Description: The following staff records shall be kept for each staff person: job title. Staff #1, employed for 3 weeks, did not have a job title documented. Plan of Correction: Provider documented the start date of staff.
