Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated and completed on 6/2/2025. The on-site inspection began at 9:50am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 10 staff. The inspector reviewed 7 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/10/2025. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.035-B-4
Description:	Repeat Violation The center is required to request a child abuse and neglect central registry search by the date of employment from any state in which staff have resided in the preceding five years. At the time of inspection, the center had not submitted an out of state child abuse and neglect central registry search request for Staff #4 who has been employed for over four months and was working alone with children.
Plan of Correction:	Background was completed but unfortunately the incorrect one for out of state. Correct one was completed and returned on date of visit.
Standard #:	8VAC20-780-280-B
Description:	Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. In the Pre-K room there was a spray bottle of bleach and odor spray stored on a lower shelf in a closet with the door open.
Plan of Correction:	Closet door was shut tightly and teacher was reminded to double recheck.

Description:	When food is brought from home, it shall be dated and labeled in a way that identifies the owner. In the Infant room there were two bottles of milk that were not dated. In the Toddler II room there was one child's food container that was not dated.
Plan of Correction:	Teachers were reminded to check labels of all students.
