Comments:

An unannounced, on-site monitoring inspection was completed on 06/03/2025, as a part of the licensure period. The on-site inspection began at 3:30 pm and ended at 4:43 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-270-A
Description:	Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Two bikes on the playground did not have pedals on the metal bars. One bike had exposed rust and the plastic seat was broken. The see-saw/rocker had exposed rust. Along the perimeter there is a 2-3" wide dip between the concrete and poured in place surfacing that creates a potential trip hazard.

Description:	Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. The cabinet in the Pooh and Friends class had multiple chemicals in the unlocked cabinet below the changing table.

Description:	The center is to maintain the staff to child ratio for birth to 16 months of 1:4. There was one staff member with 5 infants.
Plan of Correction:	Staff to child ratios will be maintained.

Description:	Prepared infant formula shall be refrigerated, dated and labeled with the child's name. There was a prepared bottle in the infant room that was not dated.
