During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 06/05/2025 and completed on 05/07/2025. The on-site inspection began at 10:00am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 11 staff. The inspector reviewed six children?s records and six staff records on-site and electronically on 06/09/2025.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
- Violations:

Standard #: 8VAC20-780-510-L Description: Medication, except for those prescriptions designated otherwise by written physician's order, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. A bottle of Children?s Tylenol for Child #6 was in an unlocked cabinet in Room #2. Plan of Correction: Corrected during the inspection.
