Ensure that personnel records clearly document staff qualifications for the position that staff are performing.
Monitor crib sheets provided by infant?s family to assure a snug fit on crib mattress.
Monitor diaper changing table cleanliness under changing pads and rough edges of tables.
Monitor playground fall zones for adequate resilient surfacing.

An unannounced, on-site monitoring inspection was completed on 04/16/2025. The on-site inspection began at 2:03 p.m. and ended at 5:25 p.m. The inspector reviewed compliance in the areas listed above. There were 41 children present and 10 staff. The inspector reviewed 5 children?s records and 6 staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/16/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition.
1. A crib railing in the Infant 2 room was observed to have peeling paint and an infant was observed in the crib.
2. Multiple plastic play structures in the playground were cracked (kitchen set and caterpillar) exposing sharp edges.
3. Playground gate had loose and expose wire creating a scratch risk to children.Plan of Correction: Crib was sanded and repainted.
Broken equipment was removed from the playground.
Gate was serviced for repair.
Standard #: 8VAC20-780-440-E Description: There shall be a least 12 inches of space between occupied rest mats. Licensing inspector observed children resting on cots in the Toddler and 2?s Room that were not 12 inches apart. Plan of Correction: Staff were reminded that children cannot be placed in feeding table without safety belts. Will check randomly.
Standard #: 8VAC20-780-500-A Description: Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Licensing inspector observed a staff member in the Infant 1 classroom changed two children?s diapers and not wash the children?s hands. The staff did not wash her hands before or after these diaper changes. Plan of Correction: Diaper changing procedures were reviewed with all staff to ensure everyone understands the requirements.
Standard #: 8VAC20-780-570-A Description: When a feeding table is being used for children, the protective belt shall be fastened securely. A child was observed at the feeding table without a protective belt being securely fastened. Plan of Correction: Cots were fixed at the time of inspection. Staff were reminded of spacing requirements.
