During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

An unannounced, on-site monitoring inspection was initiated and concluded on June 3, 2025. The on-site inspection began at 12:12 p.m. and ended at 1:30 p.m. The inspector reviewed compliance in the areas listed above. There were 23 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 16, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A Description: Repeat Violation
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The screening for Staff #1 was completed five days after hire.Plan of Correction: Ensure I receive TB results before hire.
Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all the required information. An incident report did not contain documentation of the future action to prevent recurrence. Plan of Correction: When receiving injury reports double check that all fields are filled in.
Standard #: 8VAC20-780-570-C Description: The record of each child on formula shall contain the brand of formula. The record of child #3, enrolled for six months, did not contain the brand of formula the child is on and staff did not know. Plan of Correction: Have parents disclose that info upon arrival.
