An unannounced monitoring inspection was initiated on May 22, 2025, and completed on May 22, 2025, in response to a self-report received by the licensing office on May 8, 2025, relating to procedures for emergencies. The inspector reviewed one child record. This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on June 16, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On May 7, 2025, child #1 fell on the playground sustaining a serious injury. It was reported that child #1 told Staff #1 that they heard a pop in their ankle. Staff #1 reported that they were on their way to retrieve an ice pack but was sidetracked with other responsibilities. Staff #1 did not provide care to child #1?s injury and did not provide first aid. Child #1 sat out for approximately one hour until parent picked up. Plan of Correction: Not available online. Standard #: 8VAC20-780-550-N Description: The center shall notify the parent immediately if a child sustains a serious injury. Child #1 sustained a serious injury while in care at the center on May 7th, 2025. The parent was never notified by the center of the injury. Plan of Correction: Not available online. Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required information. On May 7, 2025, child #1 sustained a serious injury to the ankle. The injury report was not completed until May 8, 2025, and did not include the following required information; treatment; Date and time when parents were notified; Any future action to prevent recurrence of the injury; Staff and parent signatures or two staff signatures; and documentation on how parent was notified.
