During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated and completed on 6/5/2025 in response to a complaint received by the licensing office on 5/29/2025 relating to allegations of forbidden actions. The Department entered into a Consent Agreement with the facility on 12/20/2024. The inspector did not review any children?s records and 2 staff records on-site. This inspection included document review, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
