An unannounced, on-site renewal inspection was completed on 05/22/2025. The inspection began at 9:40 a.m. and ended at 4:25 p.m. The inspector reviewed compliance in the areas listed above. There were 97 children present and 23 staff. The inspector reviewed 9 children?s records and 10 staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/17/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-130-E Description: Repeat Violation
The center shall obtain documentation of additional immunizations once every six
months for children under the age of two years. C8, under the age of 2 did not have documentation of additional immunizations due by 03/13/2025.Plan of Correction: Sent out email for all parents for missing information.
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening. S3 updated screening was completed 1 month late and S6 was completed 10 days late. Plan of Correction: All have been corrected.
Standard #: 8VAC20-780-40-J Description: The center is required to update annually the injury prevention procedures based on documentation of injuries and a review of the activities and services. No documentation was
available to show compliance with this requirement.Plan of Correction: Created a document for this in binder.
Standard #: 8VAC20-780-420-E-3 Description: Repeat Violation
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of C8 had not been updated in over 17 months and C9 had not been updated since enrollment in 2023.Plan of Correction: Sent out email for all parents for missing information.
