An unannounced inspection was initiated on June 10, 2025 and completed on June 11, 2025 in response to a complaint received by the licensing office on June 4, 2025 relating to allegations of forbidden actions and behavior guidance. The inspector reviewed one child's record and five staff records on-site. This inspection included document review, tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Complaint related: No Description: At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
The last documented TB screening for Staff #1 expired over ten months and the TB screening for Staff #5 expired six weeks ago.Plan of Correction: Both staff members will go today for a TB screening.
