Comments:
An unannounced, on-site monitoring inspection was completed on 06/10/2025, as a part of the licensure period. The on-site inspection began at 1:15 pm and ended at 2:45 pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 3 staff. The inspector reviewed 5children?s records and 2 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #1, #2, and #3, who had been attending for one months, did not have documentation of an immunization record on file.

Plan of Correction: Center will request information from parents.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There were no physical examinations in the records of child #1, #2, and #3.

Plan of Correction: Center will request information from parents.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The records for Child #1, #2, and #3, enrolled for one month, were incomplete.

Plan of Correction: Center will request information from parents.

Standard #: 8VAC20-780-560-G
Description: When food is brought from home the food container must be clearly dated.

None of the children's food containers were dated.

Plan of Correction: Center will ensure food brough from home is dated.
