Licensing standards regarding (8VAC20-780-510) and the renewal application were discussed with the director.

An unannounced, on-site monitoring inspection was initiated on June 5, 2025 and completed on June 5, 2025. The on-site inspection began at 11:15am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/18/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-770-70-A Description: The following staff records shall be kept for each staff person: Two or more references as to character and reputation as well as competency were checked before employment. The record for staff #1 had one reference documented and the record for staff #2 did not have documentation of at least two references. Plan of Correction: Staff #1 second reference arrived in the mail that day. Staff #2 brought theirs this week.
Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #3, under two years old has been in care for 10 months did not have additional immunizations in their record. Plan of Correction: I received child #3 immunization record 6/6/25.
Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information a work phone number of each parent who has custody and address of two designated people to call in an emergency if a parent cannot be reached. The record for child #1 who has been enrolled for approximately two years did not have the work phone number for one parent. The record for child #1 and child #2 who has been enrolled for over five months did not have the address for both emergency contacts.Plan of Correction: Child #1 parent work phone has been collected. Child #1 and #2 emergency contact addresses have been collected.
