An unannounced, on-site monitoring inspection was initiated on 6/10/2025 and completed on 6/11/2025, as a part of the licensure period. The on-site inspection began at 9:57AM and ended at 11:48AM. The inspector reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 3 children?s records on site on 6/10/2025 and 2 staff records on site 6/10/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The record for Child #3, enrolled for three months, was missing the following required components: address of two designated people to call in an emergency; previous child day care and schools attended by the child; documentation of viewing proof of child?s identity and age and first date of attendance.
