The Department entered into a Consent Agreement with the facility on April 16, 2025. An unannounced, on-site monitoring inspection was initiated and concluded on June 10, 2025, with two inspectors present. The on-site inspection began at 10:30 a.m. and ended at 12:55 p.m. The inspectors reviewed compliance in the areas listed above. There were 116 children present and 16 staff. The inspectors reviewed 7 children?s records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 25, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-M Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The allergy list in each classroom of the center was not updated to include children in care with allergies and intolerances. Plan of Correction: The center has updated the allergy, sensitivity, and dietary restriction lists in each classroom to reflect all current children in care. Each list is now dated, clearly marked confidential, and securely posted in a location accessible to staff but out of view from visitors. Going forward, classroom teachers will review and confirm the accuracy of their allergy lists weekly, and updates will be made immediately upon enrollment or any changes reported by families. Center leadership will audit lists monthly to ensure compliance.
Standard #: 8VAC20-780-60-A-9 Description: The center shall maintain a separate record for each child enrolled that contains all of the required information. The records of child #2, enrolled for over two years, child #3, enrolled for 10 months, child #4, enrolled for over three years, and child #6, enrolled for over a year, did not contain special accommodations needed. Plan of Correction: The center has reviewed and updated the enrollment records for all children to ensure required information, including any special accommodations, is accurately documented. The records for children #2, #3, #4, and #6 have been corrected to reflect current needs. Staff responsible for enrollment and file maintenance have been retrained on documentation requirements, and a quarterly file audit process has been implemented to ensure ongoing compliance.
Standard #: 8VAC20-780-280-B Description: Repeat Violation
Systemic Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. In the two year old classroom and in the three year old classroom, a sanitizing solution and a glass cleaner solution were in an unlocked cabinet.Plan of Correction: Extra cabinet locks will now be stored in each classroom so there is no delay in replacing a broken lock. All cleaning supplies were immediately secured in locked storage areas. Staff have been retrained on proper storage procedures, emphasizing that all hazardous materials must be kept in locked cabinets at all times. Classroom checks are now conducted three times daily by management to ensure compliance, and any violations will result in immediate corrective coaching.
Standard #: 8VAC20-780-440-B Description: Cribs, cots, rest mats, and beds shall be identified for use by a specific child. The cribs in Infant B were not identified for use by a specific child. Plan of Correction: All cribs in the Infant B classroom have been labeled to clearly identify each for use by a specific child. Staff have been retrained on the requirement that each sleep space must be assigned and labeled accordingly. The classroom teacher will verify crib assignments daily, and leadership will conduct weekly checks to ensure continued compliance.
Standard #: 8VAC20-780-520-B Description: If sunscreen is used it must be labeled with the child's name. There was sunscreen in the DP1 classroom that was not labeled with a child's name. Plan of Correction: The sunscreen bottle was labeled immediately with the child's name to match the authorization form already in the classroom. All sunscreen containers in the DP1 classroom have been reviewed. Staff have been reminded that all sunscreen must be clearly labeled before use and that any new sunscreen brought in by families must be labeled upon receipt. Classroom teachers will check daily to ensure all sunscreen remains properly labeled.
