Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, monitoring inspection was completed on June 11, 2025. The inspection began at 1:10 PM and ended at 3:00 PM. The inspector reviewed compliance in the areas listed above. here were 43 children present and seven staff. The inspector reviewed seven children?s records and three staff records. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/23/2025. A POC submitted after this date will not appear on the public website.

Description:	For each group of children, the center is required to maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Classroom # 2 had 12 children signed in but 11 children were physically present.

Description:	Electrical outlets are required to have protective covers that are of a size that cannot be swallowed by children. In classroom #4 there were 3 outlets that did not have covers on them.

Description:	Staff are required to wash their hands with soap and running water before and after a diaper change. In classroom #3, a staff was observed not washing their hands after a diaper change.
