An unannounced, on-site monitoring inspection was completed on 06/13/2025, as a part of the licensure period. The on-site inspection began at 10:00 am and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 51 children present and 10 staff. The inspector reviewed 10 children?s records and 8 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/20/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Repeat Violation
The center shall maintain a record for each child that contains required information.
The center did not have documentation of child updates and confirmation of up- to- date information in the child's record as required by 8VAC20-780-420 E3 for Child #1 due 6/2023.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-245-J Description: Whenever the center has agreed to administer prescribed medications, the administration shall be performed by a staff member who has completed a training and are required to be retrained at three- year intervals.

Staff #2 Medication administration training had been expired 1 year and 6 months.Plan of Correction: A full review of all MAT (Medication Administration Training) certified staff has been conducted. All members of the campus leadership team have been updated and are now fully aware of which staff members are authorized to administer medication. Effective immediately, staff who are not MAT certified will not be permitted to administer any medication under any circumstances.
Planned Follow-Up:
To ensure appropriate coverage, 2?3 additional staff members will be enrolled in a MAT certification session. The training date is currently being finalized and will be scheduled as soon as possible.
Standard #: 8VAC20-780-510-E Description: The center's procedures for administering medication shall include methods to prevent use of outdated medication.
1. Child #2 has a food allergy.
2. The emergency medication maintained at the center had expired two weeks prior to inspection.Plan of Correction: The medication in question was associated with a known allergy. As of May 13, 2025, the child was evaluated by a pediatrician who determined the allergy was no longer present. The medication was unopened and stored with the appropriate documentation at the time of inspection. Upon being notified of the expiration, the family promptly provided a physician?s statement confirming that the child no longer requires the medication. The expired medication has since been removed from the premises.
Standard #: 8VAC20-780-510-I Description: The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the medication is administered by a staff member that meets the requirements in 8VAC20-780-245J.

Standard #: 8VAC20-780-560-G Description: When food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
11 lunch boxes for children were not labeled with name and date as required.Plan of Correction: A new labeling system has been developed and will be implemented beginning July 1, 2025. Each lunchbox will be labeled with the child?s name and date to ensure clear identification and to maintain health and safety standards.
