During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated and completed on 5-27-25 in response to a self-report received by the licensing office on 5/22/25 relating to an injury. The Department issued a Notice of Intent to place the licensee on probation for a period of six months; and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations on 3/12/25 as a result of high-risk violations. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, interviews, and observations. Though not a part of the original self-report, a violation that was not reported was found related to sight and sound supervision and was documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/24/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-F Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.
On 5/22/25, in the Creme Prep classroom, Staff #1 was alone with 10 children in the tile floor side of the classroom that is divided by a half wall. Staff #1 was observed moving back and forth between the two sides of the classroom leaving the 10 children alone in the tile area when she would exit to the carpeted area. This occurred off and on for approximately 20 minutes.Plan of Correction: AU team members have been
instructed to view each side of the classroom as two rooms and will
remain on one side only. All
teachers were re,.trained on correct classroom placement, how to handle individual scenarios regarding the other side of the room and when to call for help. This training occurre\'.t on June 3, 2025. Additionally, each teacher was provided with a walkie talkie to reach a director directly.
