An unannounced monitoring inspection was initiated on June 9 2025 and completed on June 16 2025 in response to a self-report received by the licensing office on May 29 2025 relating to Care of Children. The inspector reviewed 1 child?s records and 2 staff records.This inspection included a document review, tour of the facility, interviews, and observations.
