During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on June 3, 2025, and completed on June 5, 2025, as a part of the licensure period. The inspector reviewed compliance in the areas listed above. There were 68 children present and seven staff. The inspector reviewed five children?s records and six staff records on-site on June 5, 2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-1 Description: The center shall obtain a completed sworn statement prior to the employee's first
day of employment. Staff #4, employed for two days, did not have a completed sworn statement. Staff was working at the time of inspection.Plan of Correction: Per the director: I will have staff complete the sworn statement today. Going forward, staff will complete prior to hire.
Standard #: 22.1-289.035-B-2 Description: The center shall request a search of the central registry prior to the employee's first day of
employment. 1. Staff #4, employed for two days, did not have the results of a central registry search. The record did not contain documentation of a central registry check being requested prior to employment. 2. Staff #5, employed for two days, did not have the results of a central registry search. The record did not contain documentation of a central registry check being requested prior to employment.Plan of Correction: Per the director: We will documentation the date of request and request will be made prior to hire. Results were received.
Standard #: 22.1-289.035-B-3 Description: Centers shall obtain a completed national criminal fingerprint background check prior to
the employee's first day of employment. 1. The record of staff #4, employed for two days, did not contain results of a national criminal fingerprint background check.Plan of Correction: Per the director: We will received fingerprint results prior to hire.
