During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated and completed on 06/16/2025 in response to a self-report received by the licensing office on 06/09/25 relating to supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/03/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the centers own policies and procedures that are required by the standards.
1. The center's policy titled "Playground Headcount Form Policy" states "when you are ready to bring your class back inside the center, children must line up at the playground door and be called by name then checked off the playground sheet. If you then proceed to travel through a classroom and down the hallway you must check the attendance sheet once you are in your classroom to be sure no child has strayed away or been left at the playground door."
2. On June 9th, 2025 child #1, age 3 years, was left alone and unsupervised on the playground for approximately 5 minutes and was discovered sitting on the steps by the classroom door by another staff member when they took their class out.
3. Interviews with staff #1 revealed that when the class was transitioned back into the classroom from the playground, the playground headcount form was not checked.
4. Staff #1 did not realize child #1, age 3 years, was left alone and unsupervised on the playground until approximately 10 minutes after entering the classroom when another staff member brought child #1 back in from the playground.Plan of Correction: Brightwheel Name-to-Face Checks
Staff are required to complete a Name-to-Face check using the Brightwheel app during each of the following transition times:
-When entering or exiting classrooms
-When entering or exiting the building
-Before and after nap time
-When traveling from one classroom or space to another
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

Staff #1 did not realize child #1, age 3 years, was left alone and unsupervised on the playground until approximately 10 minutes after entering the classroom when another staff member brought child #1 back in from the playground.Plan of Correction: All Teachers underwent a training on how to properly use their walkie talkies, Brightwheel, reviewed sight and sound, and head count policy.
Standard #: 8VAC20-780-340-F Description: Repeat Violation
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On June 9th, 2025 child #1, age 3 years, was left alone and unsupervised on the playground for approximately 5 minutes.Plan of Correction: Use of Walkie-Talkie Communication
-Teachers must radio the office using walkie-talkies before exiting or entering the building with children.
-Office staff must acknowledge the communication and provide visual supervision from the door or security camera system as children move through the doorway.
