An unannounced monitoring inspection was initiated and completed on 06/24/2025. The on-site inspection began at 9:05 am and ended at 10:35 am. The inspector reviewed compliance in the areas listed above. There were 56 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records.
The inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-1 Description: The center is required to obtain a completed sworn statement prior to the employee?s first day of employment.
The sworn statement for Staff 1 was dated 2 days after their first day of employment.Plan of Correction: Moving forward, will get prior to hire.
Standard #: 22.1-289.035-B-2 Description: Providers must obtain a completed national criminal background check prior to the employee?s first day of employment.
The national criminal background check was dated 2 days after their first day of employment.Plan of Correction: Moving forward, will get prior to hire.
Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment and shall have been completed within the last 30 days of employment.
There was no documentation of a TB screening for Staff 1 who has been employed for one week. The TB screening for Staff 2 was completed one week after employment.Plan of Correction: Moving forward, will get prior to hire.
