During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 6/10/25, as a part of the conditional licensure period. The on-site inspection began at 12noon and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/2/25. A POC submitted after this date will not appear on the public website.

Description: The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The current license was not posted in the center.

Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff#1, who had been employed for more than one month, did not have a completed national criminal background check prior to employment.

Description: The center may allow a child to attend contingent upon a conditional enrollment for a period of 90 days if the child received at least one dose of each of the required vaccines and the child possesses a plan for completing his immunization requirements within the ensuing 90 calendar days. The most recent immunization documentation for Child#1 is dated 1/15/24. The record of the 2 year old child did not include a plan for completing immunization requirements within the allotted time frame.

Description: Each staff member shall submit documentation of a negative tuberculosis (TB) screening that was completed within the last 30 calendar days of the date of employment. Description: The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. Description: Areas and equipment of the center, inside and outside, shall be maintained in clean and safe condition. There were plastic grocery bags stored within reach of children, posing a suffocation hazard. There was a cracked electrical outlet face plate within reach of children the child care room children were observed in. There is an HVAC vent that is open and filter sitting on the floor, all within reach of children in the multi-purpose room. There are multiple protruding nails/screws on the fence surrounding the outside play area. The portion of nails/screws that is protruding is the sharp tip of the nail/screw. There are pest droppings visible in drawers and cabinets on the second floor.

Description: In inside areas occupied by children, the temperature shall be maintained no lower than 68?F. The temperature on the first floor thermostat read 65?F. The thermostat was set for 60?F.

Description: Hazardous substances must be in a locked place that prevents access by children. There were multiple chemical containers within reach of children in the unlocked office, including but not limited to bleach wipes, hand sanitizer and spray cleaners.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff. Two preschool age children were left unattended outside the bathroom while a staff member went into the restroom, closed the door and assisted another child inside the restroom. The children were not within sight and sound supervision of any staff while the staff was in the bathroom.
