During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 5/12/2025 and completed on 6/27/2025. The on-site inspection began on 5/12/2025 at 11:40 am and ended at 2:55 pm. The inspector reviewed compliance in the areas listed above. There were 40 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. Additional documentation was received electronically on 5/13/2025. This inspection included document review, tour of the facility, interviews, observations and measurement.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/7/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-70 Description: Staff records must contain documentation to demonstrate that the individual possesses the education, certification, and experience required by their job position. There was not sufficient documentation to demonstrate that Staff #2 possess the education and certification, required by the designated job position of Program Leader for a group of children. Standard #: 8VAC20-780-350-B-4 Description: The required ratio for children ages 3 years of age up through school age eligible is one staff per ten children. Staff #4 and staff #5 were observed supervising 13 four year old children each, with staff #3 moving back and forth between the adjoined classrooms to assist.
