Comments:

An unannounced, on-site monitoring inspection was completed on 06/25/2025. The on-site inspection began at 10:32 a.m. and ended at approximately 4:38 p.m. The inspector reviewed compliance in the areas listed above. There were 62 children present and 10 staff. The inspector reviewed 7 child records and 8 staff records. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/07/2025. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-130-E
Description:	The center shall obtain documentation of additional immunizations once every six months for a child under the age of two years. C5 under the age of 2 years old and enrolled for 10 months did not have documentation of additional immunizations in the file.
Plan of Correction:	Reviewing all files. Missing documents requested by 07/11/2025.
Standard #:	8VAC20-780-140-A
Description:	Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of C4, who has been enrolled for five months.

Standard #:	8VAC20-780-150-B
Description:	Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for C3 was not signed.

Description:	Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for C4, enrolled for 5 months, was missing the documentation of viewing proof of the child?s identity and age.

Description:	Records shall be kept for each staff person that contains all of the required information. The records for S1, S2, S3, and S4 did not contain information about any health problems that may interfere with fulfilling the job responsibilities and contact information of a person to be notified in an emergency. The record for S4 contained 1 of 2 required references to character, reputation, and competency checked prior to employment.

Description:	The center shall provide an annual fire inspection report. The last inspection for the center was more than 12 months ago. The last inspection was dated 04/09/2024.

Description:	Children?s hands shall be washed with soap and water or wipes before and after eating. Multiple children were observed having finished eating their snacks in the 16-month classroom and no hands were washed afterwards.
Plan of Correction:	Spoke to teacher about proper hand washing.
Standard #:	8VAC20-780-530-A
Description:	At least one staff in each classroom where children are present shall have current certification in cardiopulmonary resuscitation (CPR) and first aid. S4 was observed in a 16-month old classroom supervising children without a current certification in CPR and first aid.
Plan of Correction:	All staff in need of CPR/First Aid took it 06/26/2025.

Description:	Children shall not be allowed to drink or eat while walking around. Licensing inspector observed two children walking with a bottles in their mouths and drinking in the 16-month classroom. A child was observed picking up food from the floor while walking around and eating it.
Plan of Correction:	Spoke to teacher and coached on proper supervision during meals.
