During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 6/16/2025 and completed on 6/28/2025. The on-site inspection began at 2:12 pm and ended at 4:50 pm. The inspector reviewed compliance in the areas listed above. There were 53 children present and 8 staff. The inspector reviewed 5 children?s records and 6 staff records on-site.This inspection included document review, tour of the facility, interviews, observations, and measurements
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-80-A
Description: (Repeat Violation). The program is required to maintain a written record of daily attendance, for each group of children, that documents the arrival and departure of each child in care, as it occurs. The written attendance record in the three year old room documented 5 children in care when 9 children were present. The written attendance record in the four year old room documented 8 children in care when 10 children were present.

Plan of Correction: The attendance sheets were updated to reflect the additional children who had come into the three year old and four year old classes to reflect the children who had joined their group for naptime. Staff will ensure that children are added and removed from their written attendance as they move from one group to another within the program.

Standard #: 8VAC20-780-270-A
Description: Areas and equipment of the center, inside and outside, must be maintained in a clean, safe and operable condition.
There was a pest control glue trap located on the floor in a classroom approximately 6 inches from a child who was sleeping on a rest mat during nap time.

Plan of Correction: The pest control trap was removed from the classroom during the inspection. Staff will ensure that these traps are never located in an area that is accessible to children in care.
