During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 6/24/25. A self-reported incident on 3/13/25 relating to supervision was also reviewed during this inspection. The on-site inspection began at 11:55am and ended at 2:30pm. The inspector reviewed compliance in the areas listed above. There were 78 children present and 16 staff. The inspector reviewed 7 children's records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. There were no violations related to the self-reported incident.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child 3 (enrolled for six months), child 4 (enrolled for over two years) and child 5 (enrolled for six months) did not have documentation of immunization records on file.

Plan of Correction: The center responded with the following: The parents will be contacted and will be given until the end of the week to turn in the missing immunization records. Children's records will be audited to ensure they contain all required information and health documents. Going forward, center management will ensure documentation of immunization records are obtained before the child begins care.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child 2 (enrolled for over a year and a half), child 4 (enrolled for over two years) or child 4 (enrolled for six months). Documentation of a physical examination for child 6 was not obtained until three months after enrollment.

Plan of Correction: The center responded with the following: The parents will be contacted and will be given until the end of the week to turn in documentation of physical examinations. Children's records will be audited to ensure they contain all required information and health documents. Going forward, center management will ensure documentation of physical examinations are obtained within the required time frames.

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for child 1 and child 2, both enrolled for over a year and a half, did not contain the work phone number and place of employment for one parent and the work number for the other parent. The record for child 3, enrolled for 6 months, did not contain the work number for one parent.

Plan of Correction: The center responded with the following: Center management will contact each parent today to obtain the missing information. Center management will audit children's records on a regular basis to ensure they contain all required information.
