During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and complete on 6/25/25, as a part of the licensure period. The on-site inspection began at 3:00pm and ended at 4:30pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and 4 staff. The inspector reviewed 4 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/7/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-130-A
Description: Repeat violation
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #3, who had been attending for over a year, had immunizations in the record that was over a year past the required timeframe.

Child #4, who had been attending for 11 months, had immunizations in the record that were 7 months past the required timeframe.

Plan of Correction: Program Director has gone through and updated all records to ensure that a Physical Exam and Immunization is completed. Moving forward Program Director will ensure that new enrollments Immunizations are on file before start of enrollment and Physical Exam completed within 30 days after the first day of attendance.

Description: Repeat violation
Physical examinations completed by a physician shall be obtained prior to first day of attendance or within 30 days after the first day of attendance.

Child #3, who had been attending for over a year, had a physical on record that was over a year past the required timeframe.

Child #8, who had been attending for over a year, had immunizations on record that were 6 months past the required timeframe.

Plan of Correction: Program Director has gone through and updated all records to ensure that a Physical Exam and Immunization is completed. Moving forward Program Director will ensure that new enrollments Immunizations are on file before start of enrollment and Physical Exam completed within 30 days after the first day of attendance.
