An unannounced monitoring inspection was initiated on May 23 2025 and completed on May 27 2025, with a local agency, in response to a self-report received by the licensing office on May 20 2025 relating to Forbidden Actions. The on-site inspection began at 9:30am and ended at 10:30am. The inspector reviewed 1 child's record and 1 staff record. This inspection included a document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 9 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff #2 admitted to pushing Child #1, 5 years old, down onto their cot. Child #1 was observed with a red mark on their back. Staff #1 witnessed the action and removed Child #1 from the classroom and told Staff #2 to leave the center.Plan of Correction: - All staff were retrained on positive child guidance on 5/22/25

Standard #: 8VAC20-780-410-1 Description: The following actions or threats thereof are forbidden: Physical punishment, striking a child, roughly handling a child.
In an interview conducted on May 23, 2025, Staff #1 stated that they observed Staff #2 pushing Child #1, 5 years of age, onto their cot during nap time. Staff #1 intervened and removed Child #1 from the classroom and took them to the office. Staff #1 observed that Child #1 had a red mark on their back. Staff #1 informed Staff #2 they needed to leave the center.
In an interview conducted May 27 2025 with a local agency, Staff #2 stated that she pushed Child #1 on to her cot due to being frustrated. Staff # 2 stated that Staff #1 spoke with them and had them leave the building.Plan of Correction: - All staff were retrained on positive child guidance on 5/22/25
