An unannounced, on-site monitoring inspection was initiated and completed on July 1, 2025, as a part of the licensure period. The on-site inspection began at 10:40 AM and ended at 2:35 PM. The inspector reviewed compliance in the areas listed above. There were 83 children present and 16 staff. The inspector reviewed 7 children?s records and 7 staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 10, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
There was poison ivy growing through the fence in several areas along the far corner of the preschool playground.
There were electrical cords for a tablet, a phone and a decorative light strewn across the sink used by staff in the preschool classroom.Plan of Correction: Provider response:
We have scheduled our maintenance personnel to come on Wednesday, July 3rd, 2025 to remove the poison ivy that was recently identified on the premises. In addition, weed killer will be applied to the affected areas after all children have left for the day to ensure their safety.
All staff were reminded that electrical cords must be managed safely at all times. Specifically: keeping all cords out of reach of children, not placed near sinks or other water sources, and ensuring that they are secured properly to prevent tripping hazards. These precautions are essential for maintaining a safe environment for both children and staff.
Standard #: 8VAC20-780-280-B Description: Repeat violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
The door to the unsupervised kitchen was open and unlocked. There was an open bucket of floor cleaner, unlocked dish detergent and sanitizer with warning labels and a container of dish sanitizer in an unlocked drawer.
There was hand sanitizer with a warning label on a shelf in the preschool classroom.Plan of Correction: Provider response:
Reminded all staff that any item considered a chemical substance, including but not limited to cleaning agents, disinfectants, sanitizers, and other hazardous materials, must be securely stored at all times. These items must be kept locked in a cabinet or drawer when not in use to ensure the safety of all individuals in the facility.
Standard #: 8VAC20-780-280-H Description: Cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children. There was a container of over the counter topical ointment on a shelf in the preschool classroom that staff stated was usually kept in the child's hanging backpack. All staff were reminded that items such as chapstick, Vaseline, and similar products, while not classified as hazardous substances, must be kept out of reach of children at all times. This is t ensure a safe environment and to prevent unintended use or ingestion.
Standard #: 8VAC20-780-430-I Description: If combs, toothbrushes, or other personal articles are used, they shall be individually assigned. There was a tube of Vaseline that was not labeled with the child's name. There was a child with a pacifier that was not labeled to identify the child to which it belonged. Plan of Correction: Provider response:
Please note that child 7 does have a pacifier lanyard; however, at the time in question, the pacifier was not attached because the child was finishing up lunch and transitioning to nap time. A mass message has been sent to all staff reminding them of the importance of keeping pacifiers securely attached to lanyards at all times, when in use. This is essential to ensuring ongoing compliance with safety guidelines.
Standard #: 8VAC20-780-500-B Description: A nonabsorbent surface for diapering or changing shall be used. Or children under three years, this surface shall be a changing table or countertop designated for changing. Staff in the two year old classroom were observed changing a child's diaper by standing the child on the floor of the bathroom. Plan of Correction: Provider response:
Due to all the children in the classroom currently potty training, we will be ordering two additional changing pads. This is to ensure that diapering and toileting procedures continue to meet the licensing standards and are carried out safely and hygienically.
Standard #: 8VAC20-780-510-E Description: The center's procedures for administering medication shall include parent authorization for the medication. There was an emergency medication for child 8 that did not have a current parent authorization for the medication. Plan of Correction: Provider response:
Reminded staff who are Medication Administration Training (MAT) to carefully review all dates on Medication Administration forms and related documents. A recent violation occurred due to a failure to verify the accuracy of dates entered by a parent. Specifically the date on the front of the form was 7/11/2025, while on the back was 6/11/2025, indicating a discrepancy that went unnoticed.
Standard #: 8VAC20-780-510-P Description: When a medication expires, the parent shall be notified that the medication needs to be picked up within 14 days. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. There was an expired emergency medication for child 8 that had not been returned to the parent or disposed. Plan of Correction: Provider response:
All staff certified in Medication Administration Training (MAT) have been reminded that any expired medication must either be sent home with the parent/guardian or renewed within 10 days of expiration. This protocol is essential to ensure compliance with medication storage regulations and to support the health and safety of all children in our care.
