During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated on June 25, 2025, and completed on July 11, 2025, in response to a complaint received by the licensing office on June 24, 2025 relating to allegations Administration, Physical Plant, Staffing and Supervision and Programs. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Special Services and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 18, 2025. A POC submitted after this date will not appear on the public website.

Description: Before leaving on a field trip, a schedule of the trip's events and locations shall be posted and visible at the center site.

On, June 23, 2025, classroom #1 left the center on a scheduled field trip, when they arrived at their destination the field trip location was closed. Classroom #1 went to a different location then originally planned. The schedule for the second location was not posted at the center site.

On June 24, 2025, classroom #2 and #3 left the center and traveled to a location that was not posted at the center site and there was no schedule available of the trip's events.

Plan of Correction: Classroom #1, 6/23, 5 minutes after arriving at the alternate site, photos were posted to our classroom management application. While a formal notice was not given, two way communication between the teachers, center, and parents was maintained throughout the entire field trip. A formal notice and apology were messaged to parents on 6/25. Our policies and procedures were reviewed and updated to include what to do when a field trip location might need to change.
Classroom #2 and #3 All off-site locations were added to our documentation that is shared with parents prior to each week if it will be used. Along with information about how and when they will be notified if groups go to any off site loactions. Two way communication between the teachers, center, and parents was maintained throughout the entire time off site.
