Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated on 07/07/2025 and completed on 07/07/2025. The on-site inspection began at 10:05 am and ended at 1:05 pm. The inspector reviewed compliance in the areas listed above. There were 15 children present and 4 staff. The inspector reviewed 6 children's records and 8 staff records on-site on 07/07/2025. The inspection included: *Document Review *Tour of the Facility *Interviews *Observations Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.011-F
Description:	Other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The inspection documents for the most recent inspection, conducted on 05/13/2025 and 05/19/2025 were not posted.
Plan of Correction:	The current inspection documents were posted during the inspection.
Standard #:	8VAC20-780-160-C
Description:	At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. The TB screening for staff 1 was not updated every two years. The most recent screening was completed more than two years from the date of the original screening.
Plan of Correction:	The Director stated the TB screening form had been misread and it was thought that the screening was still current. Staff 1 is currently on a leave of absence however, the screening will be updated when she returns to work.
Standard #:	8VAC20-780-60-A
Description:	Children's records shall contain all of the required elements: There was no written care plan for child 1 who has a diagnosed food allergy.
Plan of Correction:	The program director stated that the parent of child 1 has provided verbal instructions regarding the child's food allergies. The allergy care plan form will be given to the parent for completion by child 1's physician.
Standard #:	8VAC20-780-240-A
Description:	The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Staff 1 has not completed the Virginia Department of Education orientation course.
Plan of Correction:	The director stated that staff 1 will obtain this training when she returns to work from a leave of absence.
Standard #:	8VAC20-780-550-P
Description:	Repeat Violation The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all of the required elements. 7 of 7 injury reports reviewed lacked documentation of any future action to prevent recurrence of the injury.
Plan of Correction:	The director stated that all injury reports will include this information going forward. The forms will be updated today.
