During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced inspection was initiated on 6/26/2025 and completed on 7/08/2025 in response to a complaint received by the licensing office on 6/16/2025 relating to allegations in the areas of supervision and building maintenance. This inspection included document review, a tour of the facility, interviews, and observations. Documentation was reviewed on site and electronically on 7/3/2025.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/15/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
On 6/11/2025, poison ivy plants were identified along the edge of the fence within a preschool playground.Plan of Correction: All items have been fixed. June 17, 2025- Replaced any missing mulch with fresh, clean materials-Cleared remaining branches and debris from the site-Conducted a full inspection with our staff to confirm all ivy and hazardous material had been removed

When staff are supervising children, they shall always ensure their care and protection.
On or around 3/13/2025, Staff #1 supervised a group of preschool aged children for several hours after consuming Marijuana.Plan of Correction: Employee was written up on 3/18/2025. On 7/9/2025 center director went over company drug policy.
