During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on July 9, 2025. The on-site inspection began at 7:20am and ended at 10:35am. The inspector reviewed compliance in the areas listed above. There were 46 children present and 10 staff. The inspector reviewed seven children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.011-F
Description: The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The current license, effective 01/27/25, was not posted on the license premises. The expired license was posted in the entrance of the center.

Violation was corrected during the inspection.

Plan of Correction: Moving forward management will ensure all licensing documentation is posted within 24 hours of receipt. The expired license was corrected prior to inspector's departure.

Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, who had been attending for over a month, before documentation of immunizations was received.

Plan of Correction: Policy is in place for all registered students not to begin their attendance on campus until their school entrance health form, to include immunizations, signed and dated on page 2 and 4, is returned. This student file was created prior to new administration. During file audit by current administration it was identified that there were no immunizations and obtained them from parents immediately prior to child's next day of attendance.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. The physical examination in the record of child #1, who had been enrolled for four months, was not received within 30 days after the first day of attendance.

Plan of Correction: Policy is in place for all registered students not to begin their attendance on campus until their school entrance health form, to include immunizations, signed and dated on page 2 and 4, is returned. This student file was created prior to new administration. During file audit by current administration it was identified that there were no immunizations and obtained them from parents immediately prior to child's next day of attendance.

Standard #: 8VAC20-780-520-A
Description: Nonprescription over-the-counter drugs or skin products shall not be kept or used beyond the expiration date of the product. In classroom #1, a tube of Butt Paste diaper ointment had expired in March 2025.
Violation was corrected during the inspection.

Plan of Correction: Policy is in place for Director to monitor all over-the-counter drugs or skin products, including documentation of application when required. This violation was corrected prior to the inspector's departure.

Standard #: 8VAC20-780-550-D
Description: The center shall implement a monthly practice evacuation drill. A monthly practice evacuation drill was not conducted for January, February, March and April 2025.

Plan of Correction: Policy is in place, calendar visually present, to ensure monthly evacuation and shelter-in-place drills are held and documented.
