Comments:

An unannounced, on-site renewal inspection was initiated and concluded on July 8, 2025. The on-site inspection began at 10:104a.m. and ended at 1:25 p.m. The inspector reviewed compliance in the areas listed above. There were 47 children present and 22 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 17, 2025. A POC submitted after this date will not appear on the public website.

Description:	After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: Water supply; Sewage disposal system; and Food service, if applicable. The center did not have documentation of a Report of Environmental Sanitation from the Health Department as required.

Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The bathroom connected to classroom 3 and used by children, contained a large area of chipped and peeling paint within reach of children. There were several areas of chipped and peeling paint in classrooms 3 and 7.

Description:	Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Classrooms 3, 4, and 5 had bleach water in unlocked cabinets, out of reach of children. Two bathrooms in the hallway contained a can of Lysol spray that was within reach of children. The unlocked laundry room contained laundry detergent and a bag of ice melt that was within reach of children.

Description:	Repeat Violation Staff shall wash hands with soap and running water before and after helping a child use the toilet. Staff #4 and #7 did not wash hands after helping a child use the toilet. Staff #6 and #7 did not wash their hands before helping a child use the toilet.
