Comments:
An unannounced inspection was completed on 07/01/2025 in response to a complaint received by the licensing office on 06/30/2025 relating to allegations of administration and staffing and supervision. The inspector reviewed 2 child and 2 staff records on-site. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation is documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/18/2025. A POC submitted after this date will not appear on the public website.

The licensee shall ensure that the center?s activities are maintained in compliance with their own policies and procedures. The center requires that staff should position themselves in quadrants of the playground to ensure adequate supervision while children are playing and circulate among the children while playing. On 06/30/2025 staff did not position or circulate around the play area used by children when a child went to the street to retrieve a ball that was kicked into the street.

Plan of Correction: 1. Steps to correct the Noncompliance
Administrative staff met with teachers individually to reinforce playground supervison policy to ensure LPP's playground policy is being followed.
2. Measures to prevent noncompliance from occurring again
Teachers received additional training regarding playground supervision policy
3. Person's responsible for implementing each step and/or monitoring any preventative measures Director/ Assistant Director will ensure training is completed and that teachers are enforcing LPP's playground supervision policy.

When staff are supervising children, they shall always ensure their care, protection and guidance. On 06/30/2025, Staff 1 and Staff 2, responsible for supervising twelve school-age children, were under a canopy with some of the children at one end of the parking lot while the other children were playing soccer at the far end of the parking lot, closer to the street (35 mph speed limit). Staff were not aware that a child was headed onto the street to retrieve a ball until they observed that a car had to stop in the road.

Administrative staff met with teachers individually to reinforce guideline to ensure children's care protection and guidance.
2. Measures to prevent noncompliance from occurring again
Teachers received additional training regarding care and supervision.
3. Person's responsible for implementing each step and/or monitoring any preventative measures Director/Assistant Director will ensure training is completed and that teachers are in compliance with licensing standards

Description: On 07/01/2025 licensing inspector observed a seven-year-old child leave the playground out of the actual sight and sound supervision of staff and enter the center?s building to use the restroom.

Administrative staff met with teachers individually to reinforce licensing standard that children 10 years of age shall be within actual sight and sound
5. Measures to prevent noncompliance from occurring again
Teachers received additional training regarding sight and sound supervison.
6. Person's responsible for implementing each step and/or monitoring any preventative measures Director/ Assistant Director will ensure training is completed and that teachers are in compliance with licensing standards
