During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
8VAC20-780-40-M. The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Comments:
An unannounced, on-site renewal inspection was initiated on July 10, 2025 and completed on July 10, 2025. The on-site inspection began at10:45 a.m. and ended at 2:15 p.m. The inspector reviewed compliance in the areas listed above. There were 68 children present and 13 staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included tour of facility, record review, observations, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 18, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A spray bottle of water/bleach mixture was on an open shelf beside the changing table.

Plan of Correction: All staff have been retrained on the proper storage of cleaning materials.

Standard #: 8VAC20-780-520-C
Description: If diaper ointment or cream is used, the following requirements shall be met: These products do not need to be kept locked but shall be inaccessible to children.
Diaper ointment was found under the changing table in an unlocked cabinet.

Plan of Correction: All staff have been retrained on proper storage of diaper cream.
