An unannounced inspection was initiated on 05/20/25 and completed on 06/12/25 in response to a complaint received by the licensing office on 05/15/25 relating to allegations of supervision, programs, special care provisions, and special services. This inspection included document review, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
