During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated on 7/14/2025 and completed on 7/15/2025. The on-site inspection began at 9:30am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 94 children present and 15 staff. The inspector reviewed 10 children?s records on-site and 10 staff records on-site. This inspection included document review, tour of facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/23/2025. A POC submitted after this date will not appear on the public website

Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #8, who had been attending for six months, did not have documentation of an immunization record on file.Plan of Correction: We will adjust our processes to ensure this does not get missed again.
Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
Child #2, 17 months old, last documented immunization was over 6 months ago.Plan of Correction: We have requested the family not to return to school without updated immunizations records.

The diapering area shall have a nonabsorbent surface for diapering or changing.
The changing pad in room 2A (toddlers/two year olds) was observed to have two rips, one on each side of the pad, making the surface absorbent. The changing pad in room 2B (2 year olds) was observed to have one rip, making the surface absorbent.Plan of Correction: We will be sure to inspect changing pad from all sides and look for stitching that is coming undone on a daily basis.
