Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 07/09/2025. The on-site inspection began at 11:20 am and ended at 1:18 pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 7 staff. The inspector reviewed 6 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/24/2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-4
Description:	The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the sex offender registry check from one state for staff #1, who has been employed for 8 months and was working with children at the time of the inspection.
Plan of Correction:	The search was done that day and will be done for all moving forward.
Standard #:	8VAC20-780-60-A
Description:	The center is required to maintain and keep at the center a separate record for each child enrolled that contains all of the required information. 1. The record for child #1, enrolled for 3 months, was missing documentation of viewing proof of the child's identity and age. 2. The record for child #2, enrolled for 6 months, was missing documentation of viewing proof of the child's identity and age.
Plan of Correction:	Proof has since been checked.
Standard #:	8VAC20-780-60-A-8
Description:	A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #1, who has been enrolled for 3 months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction:	Care plan has been received since. We will ensure we have one for all food allergies moving forward.
Standard #:	8VAC20-780-270-A
Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe, and operable condition. 1. The wooden railroad tiles on the playground are splintered, cracked, and leaning. 2. Classroom #1 and classroom #2 have areas of peeling wallpaper. 3. There was a large area of standing water that was not draining properly on the playground.
Plan of Correction:	Rail tiles are being replaced. Wallpaper is being fixed. Playground is being grated.
