22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE

Comments:
An unannounced, on-site monitoring inspection was initiated on 07/16/2025 and completed on 07/22/2025. The on-site inspection began at 9:00 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 55 children and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site; additional information was reviewed electronically on 07/22/2025.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-F
Description: The center shall obtain documentation of additional immunizations once between the child?s fourth and sixth birthdays.

The record for Child 2 did not contain documentation of additional immunizations between the child?s fourth and sixth birthday.

Plan of Correction: Created a spreadsheet for when to ask parents for immunization records.

Standard #: 8VAC20-780-60-A
Description: The center shall maintain a separate record for each child enrolled that contains all of the required information.

The records for Child 1, Child 2 and Child 4 did not contain all of the required information.

Plan of Correction: Will make sure copies in the folder match the copies in the classroom for Child 2 and Child 4; Child 1 has been corrected.

Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child as it occurs.

The departure of the children in Classroom 4 was not documented on 07/15/2025. The arrival of the children in Classroom 4 was documented for 07/16/2025.

Plan of Correction: Added attendance to sign in sheets and retrained subs.

Standard #: 8VAC20-780-240-A
Description: REPEAT VIOLATION

The VDOE sponsored orientation course shall be completed within 90 calendar days of employment.

Staff 2, who has been employed for 11 months, has not completed the VDOE orientation course.

Plan of Correction: Will be completed within 30 days.

Standard #: 8VAC20-780-510-E
Description: REPEAT VIOLATION
The center?s procedures for administering nonprescription medication shall be consistent with the manufacturer?s instructions for age, duration, and dosage.

Child 6 (age 19 months) was administered Medication 1 one time on 07/03/2025. The manufacturer?s instructions for this medication required physician?s instructions for administration to children under the age of 2 years.

Plan of Correction: Added not to medication box and front of medication binder.
