Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated on 07/16/25 and completed on 07/16/2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 109 children present and 19 staff. The inspector reviewed 10 children's records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 07/24/25. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	22.1-289.035-B-1
Description:	The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff #4 was dated ten days after their first day of employment.
Plan of Correction:	Per the Center: "Staff will complete a sworn statement prior to the employee's start of employment."
Standard #:	22.1-289.035-B-4
Description:	The center is required to obtain background checks from any state in which the individual has resided in the preceding five years prior to employment. The documentation in the record of Staff #9 indicated the out-of-state sex offender registry search and the out-of-state child abuse and neglect registry search were requested nine days after the staff started employment.
Plan of Correction:	Per the Center: "The center will request background checks from any state in which the individual has resided in the preceding five years prior to the start of employment."
Standard #:	8VAC20-780-160-A
Description:	Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The TB in the record of Staff #2 was completed 41 days prior to employment and the TB in the record of Staff #7 was completed 47 days prior to employment.
Plan of Correction:	Per the Center: "Staff TB records will be dated within 30 days of employment, and also before coming into contact with children."

Description:	Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #4 began employment two days prior to when their references were obtained.
Plan of Correction:	Per the Center: "Staff references will be checked prior to the employee's start of employment."
Standard #:	8VAC20-780-550-H
Description:	The center shall maintain a record of the dates of the practice drills for one year. The center did not have documentation of the monthly practice evacuation drill that was conducted in January 2025.
Plan of Correction:	Per the Center: "Records of evacuation drills will be kept on site for one year."
