An unannounced, on-site focused inspection was initiated and completed on July 14, 2025 as a part of probation. The Department issued a Special Order on April 15, 2025, to place the licensee on probation for a period of six months; and contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The on-site inspection began at 9:16am and ended at 10:24am. The inspector reviewed compliance in the areas listed above. The inspector reviewed 0 children?s records and 0 staff records on site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 25, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-290-A-3 Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers.
Children in classroom #1 were observed near one uncovered electrical outlet that was within their reach.Plan of Correction: A protective cover was promptly installed into the outlet.
