During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 7/17/2025, as a part of the provisional licensure period. The on-site inspection began at 12:40pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 108 children present and 28 staff. The inspector reviewed 6 children?s records and 2 staff records on site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A Description: Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. 2. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Staff 2 had documentation of a TB screening that was provided at the time of employment, and was not completed within 30 days of employment. It was completed over 2 years prior to their employment.Plan of Correction: Staff will complete a tb test.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
The following items were observed:
1) disinfectant spray hanging on the edge of the diaper changing table in Room 1
2) Lysol spray in unlocked teacher cabinet in Room 1
3) disinfectant spray in unlocked tall cabinet in Room 4Plan of Correction: Corrected at the time of the inspection: 7/17/2025
1) Disinfectant spray hanging on the edge of the diaper table in room 1 was removed and placed behind a locked cabinet door.
2) teacher cabinet in room 1 was locked.
3) Tell cabinet in room 4 was locked.
Standard #: 8VAC20-780-440-E Description: There shall be at least 12 inches of space between occupied cots, beds, and rest mats.
Room 2 had 4 occupied cots that were within 12 inches of each other.Plan of Correction: Corrected at the time of the inspection 7/17/2025
1) the space/distance between occupied cots in room 4 were moved to align with the standard of 12 inches between them.
2) The standard was reviewed to assure the health, safety, welfare and wellbeing of the children enrolled for care and early learning.
Standard #: 8VAC20-780-520-C Description: If diaper ointment or cream is used, the following requirements shall be met: 1. Written parent authorization noting any known adverse reactions shall be obtained; 2. These products shall be in the original container and labeled with the child's name;
1. Room 1 had a child's diaper cream that did not have an authorization form.
2. Room 2 had a child's diaper cream that did not have an authorization form.Plan of Correction: Corrected at the time of the inspection 7/17/2025
1) OTC authorization form was completed for diaper cream for child in room 1.
2) OTC authorization form was completed for diaper cream for child in room 2.
3) Protocols and procedures were received to assure the health, safety, welfare and wellbeing of children enrolled for care and early learning.
