An unannounced monitoring inspection was initiated on June 4, 2025 and completed on July 22, 2025 in response to a self-report received by the licensing office on May 22, 2025 relating to programs. The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
