An unannounced, on-site focused inspection was completed on 07/21/2025, as a part of the provisional licensure period. The on-site inspection began at 1:43 p.m. and ended at 3:50 p.m. The inspectors reviewed compliance in the areas listed above. There were 78 children present and 18 staff. The inspector reviewed 5 children?s records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/29/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of C2, who had been enrolled for nine months. Plan of Correction: Ensure that all information is filled out prior to starting.
Standard #: 8VAC20-780-550-P Description: The center shall maintain written records of children?s injuries that contains all of the
required information. Injury reports dated 06/30/2025, 07/01/2025, 07/07/2025, 07/08/2025, 07/10/2025, and 07/15/2025 did not document the time of parental notification.Plan of Correction: Ensuring that all information is still filled out including date/time.
