During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on July 21, 2025, and completed on July 21, 2025, as a part of the licensure period. The on-site inspection began at 12:25 pm and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 15 children present and four staff. The inspector reviewed five children?s records and five staff records. This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on July 30, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years.
The most recent sworn statement for staff #5 was expired by one month.

Standard #: 8VAC20-780-270-A
Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. The carpet covering the wooden fence was detached along the sides, exposing a rusty nail and multiple screws.

Standard #: 8VAC20-780-510-E
Description: The center's procedures for administering medication shall be consistent with the manufacturer's instructions for age, duration, and dosage, for non-prescription medications. 1. Medication for child #5 stated for children under two years of age, ask a doctor for dosage. 2. The medication authorization for child #5, under two years of age did not contain dosage recommendations or authorization from a physician. 3. The medication had not been administered.

Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. 1. The medication authorization for child #4 expired 16 days ago. The medication was still on site. 2. The medication authorization for child #5 expired over four months ago. The medication was still on site.
